<i>VET-PATHS (VETeran PAnel Management Tool for High-Risk Subgroups)</i> project. The study will test an innovative informatics tool that helps primary care teams identify and proactively manage high-risk Veterans using data-driven care plans. VET-PATHS aims

 to improve access, coordination, and outcomes for Veterans with multiple chronic conditions. Dr. Ashok Reddy is co-investigator. </span></p>
