Clinical news UnBIASED patient-provider interactions [image035.jpg]Dr. Brian Wood, associate professor (Allergy and Infectious Diseases) is a co-investigator on the UnBIASED (Understanding Biased patient-provider Interaction and Supporting Enhanced Discourse) Project. The goal of the project is to develop an automated tool that can be integrated into the clinic setting or training environment to help raise clinicians? awareness of moments of implicit bias in their interactions with patients. The project combines input from members of traditionally marginalized groups, including BIPOC and LGBTQ+ individuals, to learn about biased behaviors that manifest in healthcare settings and to better understand the impact on patients. The UnBIASED team has also partnered with clinicians at Harborview Medical Center and UC San Diego clinics, who volunteered to have cameras installed in their exam rooms to record actual patient visits (with patient consent) and to have their interactions analyzed for potential indicators of bias. And this legacy continues. ?Much has changed even since I joined the clinic as a trainee in 2003, as HIV treatments have become less burdensome and better tolerated. As such, our clinical focus has expanded to include HIV prevention and treatment of comorbidities associated with an aging population of people with HIV,? says Shireesha Dhanireddy, MD, director of the Madison Clinic. ?We have also developed differentiated care delivery models to decrease barriers to care for individuals for whom the traditional clinic models have not been successful. As we look ahead, we will continue to provide comprehensive co-located services with the flexibility to meet patients where they are, such as in the community or via telemedicine services.? Research news Immunotherapy for advanced chondrosarcoma[image037.jpg] Chondrosarcoma is a cancer of the cartilage, and the primary treatment is surgery. ?As a generalization, there are no other treatments,? said Dr. Lee Cranmer, professor (Medical Oncology). But Cranmer was optimistic that immunotherapy would work for his patient, Mark Schuck, a retired fisheries biologist. Although it is not approved for Schuck?s type of cancer, Cranmer?s team was able to obtain pembrolizumab, an immunotherapy drug, on a compassionate use basis. Schuck received his first infusion in September 2021. In less than three weeks, he was able to manage his pain with over-the-counter medications; within six weeks, he was off all pain relief. ?He?s had a dramatic and unexpected response despite there being no standard treatment for his condition,? Cranmer said. ?Usually, nothing stops this disease, yet he is back to fishing, his pain went away, and he has gained weight.? Cranmer is working to develop a clinical trial for a subset of patients like Schuck. ?We now have five patients with chondrosarcoma who have taken immunotherapy and responded dramatically,? he said. Research news Spotlight on resident research: Understanding Checkpoint-Inhibitor Induced Immune-Related Hepatitis [cid:image013.jpg@01D95E69.E4D1C4F0]Aiming to better predict who is at greatest risk for checkpoint inhibitor-induced hepatitis, Dr. Erica Storm, R3, and colleagues conducted a retrospective case-control study of adults who newly received checkpoint inhibitor therapies for a variety of solid organ malignancies between 2016 and 2020 at Fred Hutchinson Cancer Center. Storm identified 97 cases of Grade 2-4 immune-related hepatitis. After adjusting for other factors that could contribute to risk of hepatitis, a liver cirrhosis was found to be associated with lower risk of immune-related hepatitis. In contrast, the presence of cancer metastases within the liver was associated with higher risk. Learn more on the residency website.
