Dr. Hirano has been serving as the Associate Program Director of this multi-disciplinary training program, and has already transformed the training program with new, innovative didactics and rotations. The Fellowship attracts well over 100 applicants every year for six positions. Faculty news [image035.jpg]New Executive Vice Chair Dr. W. Robb MacLellan, professor and head (Cardiology) has accepted the newly created position of Department of Medicine Executive Vice Chair, effective July 1, 2022. Dr. MacLellan has been the Robert A. Bruce Professor of Medicine and Cardiology Division Head since November 1, 2011. He also serves as director of the UW Medicine Heart Institute. The Executive Vice Chair will be primarily responsible for clinical strategy, optimizing clinical activity, and delivery of care. In this role, he will lead the department in decision-making on clinical priorities and long-term strategies, establish and foster strong relationships with the hospitals and UW Medicine clinical leadership, and evaluate internal approaches to optimize clinical productivity and finances. We will be launching a national search for a new Cardiology Division Head in the Fall, and Robb has graciously agreed to stay on as division head until we fill that position. In addition, he will remain the head of the UW Medicine Heart Institute until a replacement is identified. ________________________________ DOM Mentorship Committee: Call for new members The Department of Medicine is extending a call for new members to join our Mentorship Committee. Clinical news Expanding Care Services for Gender-Diverse Patients [image036.jpg]Since 2018, the Transgender and Gender Non-Binary (TGNB) Health Program has been providing gender-affirming care to UW Medicine patients. The program offers a complete spectrum of gender-affirming care. Primary care, medication management, hormone replacement therapy, reproductive and sexual healthcare, and mental health care are core services, along with psychological evaluations that insurers require to cover surgery. The TGNB team is working to expand access to their services and offer more training opportunities for faculty, students, residents and fellows. ?We know there is a growing need for gender-affirming surgeries and other types of care among the gender-diverse community here in Washington and throughout the country, and an excitement among local community members that they can access these types of care at UW Medicine,? says Dr. Corinne Heinen, clinical professor (Allergy and Infectious Diseases), clinical director for the TGNB program, and director of the UW School of Medicine?s LGBTQ Health Pathway. ?We are proud to expand our services and program to meet their needs and to help advance healthcare equity among gender-diverse people.? This is especially true for women who are already underserved by the medical system. For women experiencing homelessness and female-identified sex workers in particular, barriers to accessing healthcare services include both structural and process factors (e.g., travel costs, office hours, social stigma) and individual social and clinical factors (e.g., drug use, mental stability, lack of trust and fear in the traditional healthcare system). To reduce these barriers, Dr. Shireesha Dhanireddy, professor (Allergy and Infectious Diseases) partnered with Aurora Commons and Harborview Medical Center to create the SHE Clinic (Safe, Healthy, Empowered), a community-based resource for unhoused women and female identified, street based sex workers along Seattle?s North Aurora Avenue. Research news Care for patients who decline dialysis [cid:image006.jpg@01D892B2.3604AE20]Dr. Susan Wong, associate professor (Nephrology) is leading a study that asks kidney specialists across the United States about their experiences caring for patients who are seeking a different path, and whether palliative care is offered instead of dialysis. ?We don?t have a systemized way to track patients who choose against dialysis,? she says ?so we have no way of evaluating whether we do a good job taking care of these patients after they make that choice.? In past research, Wong found that for about 1 in 7 VA patients eligible for dialysis, a decision is made not to pursue it. This suggests that the general population of end-stage renal disease (ESRD) patients with that conservative mindset is sizable. Wong launched her survey of clinicians through the National Kidney Foundation and has heard back from more than 160 providers so far. She hopes the anecdotal findings will shed light on the extent to which conservative care is offered alongside dialysis and might hint at different ranges of care offered by practitioners across care settings. Gabriela Chiorean, Philip Greenberg, Petros Grivas (Medical Oncology) and Mary-Claire King (Medical Genetics) are included on the list of eight doctors who continue to make Seattle a hotbed of cancer research and treatment in the face of overwhelming odds, in ?They?re Here To Save Your Life? from Seattle Magazine. Chiorean leads clinical trials to discover new therapies to treat pancreatic, colorectal and other gastrointestinal cancers. She has identified new chemotherapies, immunotherapies and targeted drugs to treat those cancers. Greenberg is an internationally recognized cancer immunotherapy expert who understands that medical research is meaningless until it?s made available to those who need it. He works with companies to translate lab discoveries into therapies. Grivas is heavily involved in the advanced research of evolving treatments for several cancer types. He most recently was lead researcher on the Javelin Bladder 100 trial that resulted in FDA approval of a new treatment therapy for a type of bladder cancer. King?s research on the differences between humans and chimpanzees eventually led to the discovery of the BRCA1 gene that can cause breast, ovarian and prostate cancer. She was among the few at the time pursuing the connection between cancer and genetics. Dr. Shinohara has been a transformative force within the Division of Dermatology and a champion for equity, diversity, and inclusion efforts. She has carved a path for future generations of leaders and become a strong role model for young women and residents and fellows of color who seldom see themselves represented in the disproportionately white, male-dominated field of dermatology. Over the past five years she has advised over 18 mentees, prioritizing women, those who identify as an underrepresented medical students interested in dermatology, and those interested in health disparities. O?Hare?s work has been extremely influential in the field as it has spurred innovative interventions to make the care more person-centered, including in our own community with the establishment of the Kidney Supportive Care program at the Northwest Kidney Centers. Research news Renewed action on hypertension to improve health and reduce global inequalities [image014.jpg]Historically, hypertension control has been neglected in global and national health policies. A study published this week in Nature Medicine suggests that if 80% of individuals with hypertension were screened, 80% received treatment, and 80% then reached guideline-specified targets, up to 200 million cases of cardiovascular disease (CVD) and 130 million deaths could be averted by 2050. "We need to reprioritize hypertension care in our practices," said senior author and principal investigator Dr. David Watkins, associate professor of medicine (General Internal Medicine) and global health. ?Only about one in five persons with hypertension around the world has their blood pressure well controlled. Read more on our news site. ________________________________ Surges in influenza-like illness may herald next pandemic [image016.png]Tracking surges in influenza-like illnesses might help researchers and global health doctors anticipate future respiratory viral pandemics, according to research published in PLOS Medicine. Scouring thousands of routine surveillance data points from the World Health Organization?s Global Influenza Surveillance and Response System, a UW Medicine team evaluated time-series outliers among people who had an influenza-like illness but tested negative for influenza. The group examined the incidence of non-influenza respiratory illnesses before the first COVID-19 cases were positively identified, and found spikes in 16 of 28 countries involved in the study. Influenza-like illnesses jumped about 13 weeks before the first confirmed COVID-19 peaks in 2020. ?We really didn?t expect to see such large spikes in so many countries,? said Dr. Natalie Cobb, clinical instructor (Pulmonary, Critical Care and Sleep Medicine) and lead author on the study. ?COVID-19 took us all by surprise, and I think it would have been beneficial to have something in place to alert us that a pandemic might be coming.? They talked about opportunities and priorities, how the council can partner with the Diversity Implementation Workgroup (part of the DOM Strategic Plan), and next steps. ?This was an opportunity for us to gather together and build community for the first time in several years,? said EDI Program Manager Sean Greenlee. Staff news In memoriam: Patti Allen [image024.jpg]Patti Allen passed away peacefully yesterday, surrounded by family, after developing an aggressive cancer about a month ago. Patti retired from the University of Washington in early 2021, after over 30 years of service. ?Patti was an outstanding administrator and a great support to me because of her can-do style and her complete knowledge of UW and NIH systems and processes,? said Dr. Jan Abkowitz. ?She served the Division as our administrator for nearly 20 years and was a trusted partner and friend. We all remember her thoughtfulness along with the collection of fun plastic toys and figures (termed "doodads") on her office shelf. She told me last week that these were to assure folks were relaxed when approaching her.? A celebration of life is being planned. The Department of Medicine is excited to present the Health Systems Collective, an initiative aimed at supporting people & groups in DOM interested in improving health through systems of care. The Collective seeks to do so by providing a set of resources and services to DOM members, which will expand over time and complement a set of core programs led by faculty in selected topical areas. The Collective also hosts Collective Conversations, an interview series that amplifies and disseminates system-related work by department members and their collaborators. Research news Transformative Breast Cancer Consortium Award [image028.jpg]Researchers at the Fred Hutch, including immunotherapy expert Dr. Stanley Riddell, professor (Medical Oncology) have received a four-year, $25 million grant from the U.S. Department of Defense to learn how to kill metastatic cancer?s deadly seeds before they can sprout. The new grant mechanism is designed to ?support collaborations and ideas that will transform the lives of individuals with, and/or at risk for, breast cancer and will significantly accelerate progress toward ending breast cancer.? And this transformation must be in people's lives, ?not in the health care or research system.? Riddell, who holds the Burke O?Reilly Family Endowed Chair in Immunotherapy, will co-lead a project with translational breast cancer researcher Dr. Cyrus Ghajar that aims to use both natural and engineered immunity to neutralize disseminated tumor cells. This team will use deep precision profiling of primary tumors, disseminated tumor cells and the immune microenvironments ? the immune cells in and around tumor cells ? to identify candidate antigens (markers on tumor cells that immune cells could be programmed to target) for engineered T-cell immunity.
